Conditional fields in forms and Reports

Hi,

Is there a way to set up conditional fields in a form? For example, when a value is selected from a list in a field (such as the status of an Action Plan) other fields appear or certain fields become mandatory depending on the selection made, is this possible?

It would be a great help if this functionality were also available in the reports, as these can also be used to modify elements.
